Does Creatine Cause Hair Loss?

Current evidence does not show that creatine causes hair loss. A 2025 trial measured hair directly, but it was short and limited to healthy young men.

Does creatine cause hair loss? Current evidence does not show that it does. The first randomised trial to measure hair follicles directly found no difference in hair density, follicular units, or cumulative hair thickness after 12 weeks of 5g creatine monohydrate a day. That is reassuring, but one short study in healthy young men cannot settle every long-term question.

The rumour did not come from a study where people lost hair. It came from a small 2009 trial that reported a rise in dihydrotestosterone, usually shortened to DHT. Hair was never measured. The link from "DHT changed" to "creatine makes you bald" was an assumption that travelled much further than the original data.

Why people think creatine causes hair loss

In 2009, researchers studied 20 college-aged male rugby players. Participants used a seven-day loading phase of 25g creatine per day followed by 5g per day for another 14 days. The researchers measured testosterone and DHT, not hair growth or shedding.

DHT rose during the creatine condition, while testosterone did not change. DHT is involved in androgenetic alopecia, also called male or female pattern hair loss, in people whose follicles are genetically sensitive to it. That made the result worth investigating. It did not prove hair loss.

The study had several limits:

  • Only 20 young male rugby players took part.
  • The intervention lasted three weeks.
  • It used a high loading phase before the maintenance dose.
  • It did not measure hair density, follicle health, shedding, or baldness.
  • The DHT finding was not enough to show what happened at the scalp.

For years, this was the main piece of human research behind the claim. Reviews kept pointing out the same gap: a hormone result was being treated like a hair-loss result.

Does creatine cause hair loss in the 2025 trial?

A 2025 randomised, double-blind, placebo-controlled trial finally measured hair. Researchers recruited 45 resistance-trained men aged 18 to 40 and assigned them to 5g creatine monohydrate or a maltodextrin placebo each day for 12 weeks. Thirty-eight participants completed the study.

The researchers tested total testosterone, free testosterone, and DHT. They also used a trichogram and imaging system to measure hair density, follicular-unit count, and cumulative hair thickness. There were no significant differences between the creatine and placebo groups for DHT or any hair outcome.

The 2025 result is the best direct evidence available because it tested the outcome people care about. It also used a common maintenance dose without a loading phase. It weighs more heavily on the hair question than a hormone-only study.

What the new trial cannot prove

No single study proves that an effect is impossible. The trial ran for 12 weeks, involved healthy resistance-trained men, and had a modest sample. It did not establish what happens over years, at higher doses, in women, in older adults, or in people selected for a strong family history of pattern hair loss.

The researchers did not record family history, which they identified as a limitation. They also noted that a longer trial or higher-dose protocol could produce a different result. There was no trend toward harm in the measured outcomes, but absence of a signal over 12 weeks is not a lifetime guarantee.

So, does creatine cause hair loss? The fair answer is that the current human evidence says no, while the direct evidence is still limited. Claims that it definitely makes people bald go beyond the research. Claims that no person could ever notice a problem also go further than the data can support.

DHT is part of the story, not the whole diagnosis

Pattern hair loss depends on genetics and follicle sensitivity, not one blood result by itself. Blood DHT can be discussed as a possible mechanism, but it is not a direct count of what is happening in each scalp follicle.

The 2025 trial matters because it measured both sides. DHT did not differ between groups, and neither did hair density, follicular units, or thickness. That makes the old chain of assumptions much harder to defend.

Training phases can also arrive with calorie changes, rapid weight loss, poor sleep, or a new stack of products. Creatine may be the easiest new item to blame, but it is not the only variable.

If you notice thinning, a widening part, a receding hairline, bald patches, scalp inflammation, or sudden heavy shedding, see a GP or dermatologist. A proper history and scalp assessment can separate pattern loss from shedding and other causes. Early diagnosis matters for conditions where treatment can slow further loss.

Should you stop creatine if you are worried about your hair?

If you are asking does creatine cause hair loss before deciding whether to start, you do not need to avoid it solely because the internet repeats the claim. The direct trial did not find a harmful effect. Whether the remaining uncertainty is acceptable is still your call, especially if hair loss runs strongly in your family or the concern is affecting your wellbeing.

If shedding starts, document the timing, dose, other supplements, diet changes, recent illness, stress, and family history.
